RECONSTRUCTION OF SKIN DEFECT OF DIGITS BY USING GROIN FLAP

Abstract
Injuries of hand and finger associated with defect are commonly encounted in clinical practice.

Traditionally, skin coverage has been provided by split-thickness skin grafts,

full-thickness skin grafts, local pedicle flap and distant pedicle flap or a combination of these methods.

Skin coverage should de restorde in the simplest, fastest and most reliable manner.Among those methods, the groin flap is most useful in the cases that microsurgical replantations were contraindicated or failed.

The groin flap, an axial pattern flap supplied by the superficial circumflex iliac artery, has proved to be extremely versatile in covering defect on the hand. The advantages are easy large length, tubing of the pedicle, and closure of the donor.

We have experienced 28 cases of groin flap.

The following results are obtained with good outcome in skin coverage of the bared bone and tendons.

1. Debridement should be used completely not to remain the any injuried tissue on the recipient site.

2. Tension should be avoided by the sufficient size of flap enough to cover the raw surface of the bared bone and tendons.

3. Flap should be divided more than traditional period in clinical practice.

4. The donor site should be repaired by the diagonal closure.

5. Early coverage should he done when microsurgical replantations are failed.
